Our Early Supported Discharge Service, Hoki ki te Kainga, focuses on providing high-quality, relationship-centered care to older adults recovering from illness, injury, or surgery.
About the Role
In this part-time position, you will be an integral member of the Hoki ki te Kainga team, working under the guidance of registered health professionals. Your primary responsibilities will include delivering restorative rehabilitation and daily living support to our clients. This may involve assisting with walking, showering, dressing, meal preparation, and implementing prescribed exercise programs. Your role is crucial in empowering individuals to regain their independence and confidence.
You will work in a rotating schedule of rostered shifts, which include mornings from 7:30 AM to 4:00 PM and afternoons from 11:00 AM to 7:30 PM, Monday through Sunday, including public holidays. Rosters are provided monthly, and shifts will vary.
In addition to supporting our clients, you will collaborate closely with their whanau, encouraging them to take an active role in the recovery process.

The Hoki ki te Kainga team is dedicated to facilitating early discharge and rehabilitation, allowing individuals to recover in the comfort of their own homes. We are committed to ensuring a smooth transition from hospital to home, providing timely and appropriate support to older adults across the community.
