About the Role
We are currently seeking a Registered Nurse with a unique blend of experience, passion, and enthusiasm to join our remarkable team. This 0.6 FTE role offers flexible working hours between Monday and Friday, as well as the option to work from home, allowing you to maintain a healthy work-life balance. As a Clinical Coordinator, you will find that no two days are alike. Your responsibilities will include:
- Managing a caseload of clients
- Assessing clients using InterRAI assessment tools
- Identifying support and rehabilitation needs
- Developing individualized support plans to ensure the needs of our clients and their families are met with care
- Providing clinical oversight to our support workers
- Building relationships, identifying opportunities, collaborating with other healthcare providers, and thinking creatively
Requirements
To succeed in this role, you will need to demonstrate:
- A commitment to manaakitanga and aroha, embodying respect, generosity, compassion, and warmth
- Excellent clinical experience and assessment skills
- Intermediate proficiency in Microsoft Office
- Strong organizational skills and the ability to manage competing demands
- A collaborative team spirit
- The ability to handle difficult conversations and communicate effectively with individuals from diverse backgrounds
