About the Role
We are currently seeking a Community Clinical Coordinator for a permanent, full-time position at Dunedin Hospital with a salary of $116,390. This role is crucial in supporting both inpatient and community healthcare teams to deliver efficient, effective, and safe services. As our health system undergoes exciting and innovative changes, the Community Clinical Coordinator will play a key role in ensuring that patients receive high-quality care in their homes and preventing unnecessary hospital admissions.
In this position, you will collaborate with various teams, including community teams, inpatient teams, ARC facilities, primary care agencies, unit managers, and other clinical coordinators. Your responsibilities will include:
- Providing clinical leadership and direction to staff across community services
- Monitoring and mitigating risks associated with clinical service delivery
- Coordinating ACC contracts and engaging in quality initiatives
- Managing the day-to-day operations of the service to ensure the delivery of safe, patient-focused care
- Supporting staff education and development
While this role does not include budget responsibilities, you will have the authority to manage staff within the community team.
