About the Role
We are seeking a Community Clinical Coordinator to join our team at Dunedin Hospital in a permanent, full-time capacity. The salary for this role ranges from $111,240 to $117,420 per annum, along with contributions to KiwiSaver.
In this role, you will support both inpatient and community teams in coordinating efficient, effective, and safe services within our available resources. Your primary focus will be on patient flow, ensuring that individuals receive the necessary support to return home and live quality lives.
Initially, your responsibilities will include establishing clear systems to enhance patient flow between the hospital and community settings, particularly regarding ACC contracts. You will also coordinate interim care into aged residential care (ARC) facilities and foster relationships with these facilities.
You will collaborate with various stakeholders, including community teams, inpatient teams, ARC facilities, primary care agencies, unit managers, and other clinical coordinators. As our modes of care evolve, this role presents opportunities for growth and adaptation.
In this position, you will provide leadership for clinical service delivery, offer clinical direction to staff, and ensure the safety and quality of patient-centered care.
Key Responsibilities
In your role, you will:
- Provide clinical leadership and guidance to staff across the community.
- Monitor and mitigate risks associated with service delivery.
- Coordinate ACC contracts and engage actively in quality initiatives.
- Manage the day-to-day operations of the service to ensure safe, patient-focused care.
- Support staff education and development.
While this position does not involve budget management, you will have the authority to manage staff within the community team.
