    Case Manager - Registered Nurse, Community Older Persons Mental Health Join us in delivering the best possible health outcomes for our elderly patients. We are offering a permanent position working 48 hours per fortnight (0.6 FTE) at Health New Zealand | Te Whatu Ora, an organization committed to the principles of Te Tiriti o Waitangi and dedicated to building a health system that serves all New Zealanders. About the Role As part of the Older Persons - Mental Health Community Team, you will provide psychiatric and geriatric care to older members of our community. Our goal is to deliver high-quality care to patients with mental health needs and/or complex age-related health conditions. We work closely with patients, their families, and community supports to achieve the best health outcomes. Based in Richmond, our team offers short-term case management for individuals of any age with dementia who exhibit significant behavioral symptoms, as well as those aged 75 or older with mental disorders. We emphasize fostering autonomy, independence, and quality of life while recognizing the vital role of whanau in our clients' well-being. As a case manager, you will oversee medication responses and tolerances, provide non-pharmacological support, and connect patients with broader community resources. Our community team serves as the gateway to inpatient services, allowing us to explore various community-based interventions before considering inpatient admission. We collaborate closely with the inpatient team to ensure holistic care and support during discharge. Our multidisciplinary team includes case managers, a Clinical Nurse Specialist, an ARC liaison nurse, an Occupational Therapist, a Social Worker, a Psychologist, an Old Age Psychiatrist, and a Physiotherapist. Our services operate Monday to Friday, from 8:00 AM to 4:30 PM. About You To be successful in this role, you will hold a current NCNZ registration and practicing certificate with no conditions that exclude the Mental Health Scope of Practice. A minimum of two years of post-registration nursing experience is desirable. You should possess knowledge of the following: - Physical and mental disorders commonly experienced by older persons - Contemporary treatment approaches, including person-centered care, sensory modulation, and therapeutic use of psychiatric medications - Community-based case management - Mental health assessment and care planning - Trauma-informed care - Mental Health (Compulsory Assessment and Treatment) Act, 1992 - PPPR Act - Welfare Guardianship Your skills should include: - Well-developed interpersonal skills - A demonstrated ability to work in partnership with vulnerable populations - Effective collaboration within a multidisciplinary team and the ability to work autonomously - Competence in working with patients who have complex and diverse needs Working at Health New Zealand At Health New Zealand | Te Whatu Ora, we are dedicated to providing excellent healthcare for the people of New Zealand.
