About the Role
We are offering a permanent, full-time position within our Forensic Community Team. In this role, you will manage a caseload of clients and engage in various responsibilities, including:
- Conducting multi-disciplinary team assessments, treatment planning, reviews, and discharge planning.
- Providing a social work perspective to the team, focusing on systemic and psychosocial factors that impact tangata whaiora (people seeking wellness) and their whanau (families).
- Contributing to reports and completing psychosocial assessments for tangata whaiora.
- Offering support and acting as a liaison with whanau.
- Performing risk screenings and safety planning related to child protection and family violence matters.
- Safeguarding patient rights by maintaining a comprehensive understanding of relevant legislation.
- Promoting equity in service provision for Maori tangata whaiora and their whanau.
- Maintaining knowledge of support services and government agencies that assist tangata whaiora in community living.
- Completing necessary documentation and data entry to ensure safe practice.
- Upholding professional boundaries and participating in supervision to enhance social work practice.
About the Team/Service/Location
Our Specialist Mental Health Services team is interdisciplinary and is an integral part of Te Whatu Ora - Waitaha (Canterbury). The Community Forensic Team, based at Hillmorton Hospital, provides assessment, treatment, education, training, and community-centered care for tangata whaiora with mental disorders who have offended. We pride ourselves on being a supportive, small multidisciplinary team that follows an assertive outreach model of care. This allows you to build relationships not only with your own clients but also with those under the care of the team.

About You
We are looking for candidates who possess the following skills:
- Whakamanakitanga: A practice that enhances mana and respects the tino rangatiratanga (self-determination) of individuals.
- Whakawhanaungatanga: The ability to build strong relationships and collaborate effectively with tangata whaiora, their whanau, and the multi-disciplinary team.
- A comprehensive understanding of social work perspectives and models that address the holistic needs of tangata whaiora and their whanau.
- Familiarity with Te Tiriti O Waitangi and its implications for equitable practice.
- Strong organizational skills to prioritize and manage workloads based on service needs.
- Technical literacy and proficiency in data entry to ensure safe practice.
To be considered for this role, you will need:
- Registration with the Social Work Registration Board and a current annual practice certificate. For those with limited clinical mental health experience, additional training will be required as part of your employment and will be provided.
