About the Role We are seeking a Clinical Pharmacist Facilitator to join our team on a fixed-term, part-time basis, working 64 hours per fortnight (0.8 FTE). In this role, you will integrate clinical pharmacy practices into primary care, ensuring the quality, safety, and effective use of medicines. You will collaborate closely with general practice teams, aged care providers, and community health services to conduct independent clinical medicine reviews, implement best practice guidelines, and promote continuous quality improvement. This position is ideal for a pharmacist who is passionate about primary care innovation, patient-centered care, and achieving equity in health outcomes. About the Team/Service/Location As part of the Clinical Pharmacist Facilitation team, you will be based in Wairoa, where you will work in collaboration with general practices, Health Hawke's Bay PHO, and Te Whatu Ora colleagues. Wairoa is a rural town located in northern Hawke's Bay, situated halfway between Gisborne (one hour north) and Napier (one and a half hours south). The town has a population of around 5,000, with an additional 4,000 residents in the surrounding district. The community is served by a multi-disciplinary health center that includes an integrated general practice, Wairoa Hospital, and Allied Health services. Wairoa Hospital features a two-bed emergency room, an 11-bed acute care ward, and three maternity beds. The town is known for its close-knit community and offers a variety of local shops and cafés, including East End Café and Osler's Bakery. Residents enjoy access to a community center that features a heated pool, gym, aerobic classes, and a ropes course. The Wairoa River is renowned for water skiing and rowing, while the river mouth and local beaches are popular spots for surfing and fishing. The Mahia Peninsula, just a 30-minute drive away, is famous for its stunning beaches, excellent surfing breaks, fishing, and scuba diving. Additionally, the Morere Hot Pools are located approximately 30 minutes northeast of Wairoa. Hawke's Bay offers a fantastic work-life balance, with sunny weather, beautiful beaches, outdoor adventures, great schools, and local wineries. About You To be successful as a Clinical Pharmacist Facilitator, you will need: - Registration with the Pharmacy Council of New Zealand and a current Annual Practicing Certificate with no restrictions. - A relevant clinical post-graduate qualification, or you should be working towards one, or have sufficient work experience that demonstrates general pharmaceutical knowledge in core areas, along with specialist knowledge in managing long-term conditions. - Experience in medication review services, such as Medicines Therapy Assessment or Comprehensive Medicines Management. - Skills in research, interpretation, and provision of medicine information. - A current driver's license.
