About the Role
We are seeking a Community Nurse to join our team, with an emphasis on the restorative model of care. In this role, you will work collaboratively with clients and their families to promote the highest level of independence possible. Your responsibilities will include:
- Operating from our Timaru office and managing the geographical region of Timaru and its surrounding areas, including Geraldine and Waimate.
- Conducting social and clinical assessments.
- Developing, overseeing, and regularly reviewing support plans (InterRai).
- Mentoring, verifying, and supervising support workers.
- Collaborating closely with Registered Nurses, Care Coordinators, and Support Worker colleagues.
- Engaging with funders and other service providers.
- Managing an evolving caseload that covers various contracts.
- Focusing on holistic care coordination with less emphasis on hands-on clinical nursing tasks.
This position is full-time, scheduled from 8:00 AM to 4:30 PM, but we are open to considering a 0.8 FTE arrangement if that better suits your needs.
About You
We are looking for a candidate who is an excellent communicator and can easily establish trust and relationships with clients, family members, key referral organizations, and colleagues. You should enjoy working autonomously while also valuing teamwork and collaboration.
The ideal candidate will be adaptable and flexible, as no two days will be the same, despite having set hours. You should possess strong organizational and time management skills, with the ability to adjust priorities and adapt your schedule as needed.
Being technically savvy is essential, as you will be using a laptop and mobile phone to work both online and offline while on the go. A full driver's license is required for this position.
Due to the autonomous nature of this role, we seek candidates with prior experience in the home and community care sector, or who have District Nursing, Long-Term Care, or medical nursing experience. This position may not be suitable for recent graduates or nurses with limited experience in New Zealand.
Benefits
In return for your expertise and commitment, we offer stable working hours from Monday to Friday, 8:00 AM to 4:30 PM, with some flexibility to work from home once you are established in the role.
You will receive an hourly rate ranging from $35.96 to $43.28, which is scheduled for review in October. You will also have access to a vehicle for work-related purposes, which is particularly beneficial if you live in a rural area.
