About the Role
We are currently seeking a Speech Language Therapist to join our Advancing Wellness at Home Initiative (AWHI) team. This community-based position includes some responsibilities in the inpatient setting, where you will identify patients who could benefit from our services. In your daily work, you will collaborate with an interdisciplinary team to support patients in their homes as they work towards their rehabilitation goals.
Our model emphasizes interprofessional practice, allowing you to engage in rehabilitation interventions that may typically fall under other disciplines. With our support, you will develop your clinical reasoning skills to tailor assessments and create holistic treatment plans for your patients. For instance, following an assessment, you may provide support related to loss of independence, education on managing lifestyle risks, and assistance with a basic exercise program.
As part of the role, you will also participate in ward multidisciplinary team meetings, assist with discharge planning, and meet patients on the ward to help establish their goals for home. To provide a 7-day-a-week service, clinicians in this role will be scheduled to work one weekend day per month, along with a rostered day off in the same fortnight.
About the Team/Service/Location
The AWHI team at Hutt Hospital supports individuals transitioning from hospital after experiencing a stroke, injury, or acute medical conditions. You will be joining a friendly and enthusiastic team composed of various professionals, including Nurses, Physiotherapists, Occupational Therapists, Speech Language Therapists, Social Workers, and Allied Health Assistants.
We prioritize ongoing professional development and offer opportunities for inter-professional practice training, professional support, and discipline-specific training through relevant leaders. Cultural professional supervision is also available from Māori and Pacific Health teams. As our team continues to grow, there will be opportunities for innovation and service development.
About You
We are looking for a Speech Language Therapist who is eager to expand their skill set in an interprofessional role while sharing their expertise with the team. You should have current registration and an Annual Practicing Certificate (APC), along with recent clinical experience in stroke, neurology, or community rehabilitation specialties. Ideally, you will possess a minimum of three years of experience and hold a current New Zealand driver’s license.
You must demonstrate the ability to build rapport with patients and whanau from diverse backgrounds, alongside a solid understanding of the health needs of our Māori and Pacific Island communities. A flexible approach to working in a fast-paced environment is essential, as is an understanding of interprofessional practice.
