About the Role
We are currently seeking a Clinical Needs Assessor for a part-time, permanent position based in Whangarei, with consideration for candidates from the Bay of Islands or Dargaville areas. This role involves working 32 hours per week, Monday through Friday, at a 0.8 FTE capacity.
In this position, you will be responsible for screening, triaging, and coordinating services based on comprehensive assessments. Your primary focus will be on individuals aged 65 and over, as well as those aged 16-64 who have age-related disabilities or long-term chronic health conditions. Assessments will be conducted both in clients' homes and via phone, and occasional travel throughout Northland will be required to complete home assessments.
About the Team/Service/Location
The Needs Assessment Service Coordination (NASC) team provides vital support for older individuals and those with chronic health conditions who require long-term assistance. We also cater to people of all ages who need acute short-term community home support.

About You
We are looking for a dedicated individual who is passionate about supporting people with age-related disabilities and complex health needs. The ideal candidate will possess strong verbal and written communication skills, which are essential for success in this role.
Key requirements include:
- A tertiary qualification in Nursing, Occupational Therapy, Social Work, or Physiotherapy, along with a current practicing certificate.
- Recent clinical experience that demonstrates strong assessment skills.
- Competency in te ao Māori, tikanga, and te reo Māori, or a willingness to begin this learning journey.
- A current, clean full New Zealand driver's license.
- Advanced computer skills, including proficiency in Microsoft Excel, Word, and Outlook.
- While prior experience working in the community and knowledge of services for older individuals and the disability sector is desirable, it is not mandatory to apply.
