About the Role
We are seeking a Complex Discharge Coordinator (CDC) to facilitate and coordinate the safe and timely flow of complex patients from their acceptance into the hospital through to their discharge. This role requires effective interdisciplinary communication and collaboration with patients, their families or whanau, internal staff at Te Whatu Ora Taranaki, and external organizations.
The CDC will ensure that patients classified as complex receive timely, coordinated, and proactive clinical interventions along with robust discharge planning. This is crucial for facilitating efficient, patient-centered discharges. The responsibilities include coordinating patient care across the continuum, from pre-hospital admission to discharge from hospital services.
The role emphasizes delivering high-quality, patient-focused services while maintaining efficient, cost-effective patient pathways. This includes decreasing the length of stay, optimizing bed utilization, and ensuring effective discharges before 11 am. The position entails significant coordination, including facilitating management plans and multidisciplinary meetings.
About You
We are looking for an experienced and competent nurse who meets the following criteria:
- A Registered Health Professional with a current Annual Practicing Certificate
- Expert PDRP (or equivalent competency)
- A solid understanding of and commitment to discharge planning and coordination
- Excellent interpersonal skills and the ability to communicate effectively
- Proven leadership abilities, along with strong time management, adaptability, and flexibility
- Conflict resolution skills
- Capacity to engage and liaise effectively with a variety of internal and external service providers
- Excellent organizational skills in coordinating patient care
- Competency in te ao Maori, tikanga, and te reo Maori, or a commitment to starting your learning journey in these areas
- Experience with projects or initiatives that embody the principles of Te Tiriti and the application of Matauranga Maori and Kaupapa Maori approaches, particularly in healthcare settings

This role is essential for ensuring the quality of patient pathways and discharges for complex patients. The Complex Care Coordinator is a vital member of the Senior Nursing team, contributing to collaborative teamwork and individual accountability for maintaining accurate data.
