Hillmorton Hospital, located in Christchurch, is seeking a dedicated professional to join our West Adult Community Mental Health Team. We are committed to upholding the principles of Te Tiriti o Waitangi and aim to build a health system that serves all New Zealanders effectively.
About the Role
We have a permanent part-time position available at 0.8 FTE. The West Adult Community Mental Health Service is one of four city-based teams that provides assessment and treatment services for individuals aged 18 to 65 who are experiencing mental health issues. Our work involves close collaboration with clients and their families/whānau, both within the community and alongside inpatient services.
About You
In this role, you will have the opportunity to work with a skilled and motivated multidisciplinary team. Your primary responsibility will be to provide comprehensive case management in partnership with clients, their families/whānau, and carers, with an emphasis on facilitating a smooth transition back to primary health care after receiving specialist input.
Your responsibilities will include:
- Managing a diverse case portfolio and providing oversight for each case.
- Conducting assessments of consumers and developing tailored interventions in collaboration with our multidisciplinary team.
The ideal candidate will possess the following skills and qualifications:
- A strong background in mental health is essential; experience in case management is preferred.
- The ability to triage and prioritize patients effectively and in a timely manner is crucial.
- A minimum of two years of experience in a mental health setting is preferable.
- A current New Zealand driver’s license is required.
- Registration with the Nursing Council of New Zealand, along with a current APC and a mental health scope of practice.
