We are currently seeking a dedicated professional to join our West Adult Community Mental Health Team in a permanent, part-time position at 0.8 FTE.
About the Role
The West Adult Community Mental Health Service is one of four city-based teams that provide assessment and treatment services for individuals aged 18 to 65 who are experiencing mental health challenges. Our team works closely with clients and their families or whanau, both in the community and in collaboration with inpatient services.
In this role, you will have the opportunity to work alongside a skilled and motivated multidisciplinary team. You will be responsible for providing comprehensive holistic case management in partnership with clients, their families, and caregivers. A key focus of this position is to facilitate a smooth transition for clients back to the primary healthcare sector after receiving specialized input.
Your responsibilities will include:
- Managing a varied case portfolio and overseeing case management tasks.
- Conducting assessments of consumers and developing tailored interventions in collaboration with our multidisciplinary team.
Requirements
To be successful in this role, you should possess the following skills and qualifications:
- Essential experience in the mental health field; case management experience is preferred.
- The ability to effectively triage and prioritize patient needs in a timely manner.
- A minimum of two years of experience in a mental health setting.
- A current New Zealand driver's license.
- Registration with the Nursing Council of New Zealand, including a current annual practicing certificate (APC) and a mental health scope of practice.
