About the Role
We invite experienced mental health nurses to consider a senior clinical role within our Mental Health Services for Older People (MHSOP) Community team as a Clinical Nurse Specialist. This position offers a unique opportunity to lead the integration of services across primary and secondary care for older adults.
In this advanced nursing role, you will be instrumental in enhancing the collaboration between MHSOP and the primary care sector. Your focus will be on improving access and care coordination for older individuals experiencing mental health challenges. You will work to bridge the gap between primary, community, and specialist services by developing effective communication pathways, fostering clinical integration, and ensuring that the care provided is recovery-oriented and whānau-centered.
You will be part of a virtual team of Clinical Nurse Specialists, reporting to the Nurse Lead for Integrated Care and Team Manager, and working together to deliver equitable, high-quality care across the non-urgent mental health pathway.
Key Responsibilities
- Build and sustain strong partnerships with general practitioners, practice nurses, non-governmental organizations, locality groups, and primary health organizations.
- Conduct comprehensive mental health assessments and lead appropriate care planning and referrals.
- Provide expert consultation and education to primary care teams to enhance mental health literacy.
- Facilitate wellness planning and health promotion initiatives for older adults and their whānau.
- Lead or contribute to clinical quality initiatives, projects, and service integration.
- Participate in cross-sector forums, strategy development, and service improvement activities.
- Act as a key contact for non-urgent referrals, ensuring timely triage and coordinated care.
Salaries will be aligned with the PSA Mental Health Public Health Nursing Collective Agreement, based on the Senior Nurse Scale.

Essential Requirements
- NZ Registered Mental Health Nurse
- Current Annual Practicing Certificate
- A minimum of 5 years of post-registration experience in Mental Health, with some experience in a community mental health team
- Relevant postgraduate qualification completed or nearing completion
- A full NZ driver's license with no limitations
