About the Role
We are currently looking for a Social Worker to join our Social Work team within the Older Persons Health and Rehabilitation in-patient setting. This role is crucial to our collaborative health system, where we work together to achieve positive health outcomes for patients, their families, and communities.
As a Social Worker, you will be dedicated to empowering individuals within their social context, supporting client self-determination and autonomy. Our team values respect and integrity, and we are committed to delivering high-quality and accountable services.
In this position, you will be responsible for:
- Following social work protocols developed for inpatient assessment, rehabilitation, and related community areas of work.
- Conducting bio-psych-social assessments, interventions, and discharge and rehabilitation planning in line with professional social work standards and service requirements for referred cases.
- Addressing health and disability needs that affect the well-being of individuals and their families.
- Undertaking statutory functions and responsibilities in accordance with legislative requirements, which may include child protection, mental health services, and support for vulnerable adults.
- Providing consultation services to the service and community.
- Executing social work interventions, responding to requests for information, and delivering reports within specified timeframes.
Requirements
To be successful in this role, you will need the following skills and qualifications:
- A flexible and adaptable approach to your work.
- Experience with PPPR applications.
- A solid understanding of rehabilitation processes.
- Familiarity with hospital processes and procedures.
- Knowledge and experience with IPV/EA.
- Strong interpersonal skills, with the ability to form relationships with clients and their families during stressful times.
- Competency in te ao Māori, tikanga, and te reo Māori, or a commitment to beginning your journey in these areas and taking ownership of your learning and development.
- Registration under the Social Workers Registration Act 2003, along with a current APC.
- A minimum of 2 years of experience in Physical and Mental Health as a practitioner.
Due to the urgent needs of our service, we are seeking candidates who can ideally start on short notice.
