We operate under Health New Zealand - Te Whatu Ora, which oversees the national health service and manages operations at local, district, regional, and national levels. About the Role We are seeking a Clinical Nurse Specialist (CNS) in Gerontology to join our Community Geriatric Service. In this role, you will leverage your expertise in clinical assessment and care for older adults across various settings, including community health, aged residential care, and the Emergency Department. You will lead and provide essential clinical support to other health professionals, ensuring that older adults with complex health needs receive timely and appropriate care to optimize their health outcomes. As part of a multi-disciplinary team (MDT), you will collaborate closely with clinical staff and patients and their families to deliver patient-centered, evidence-based care. We are looking for someone with proven clinical skills, educational experience, and the ability to coach and mentor others, along with strong interpersonal capabilities. Key Responsibilities - Provide expert nursing knowledge and assessment for older adults across the care continuum, including community settings, aged residential care, and the Emergency Department. - Lead clinical care for complex patients, ensuring timely assessments and follow-up. - Offer clinical support to Community Central, Virtual Clinics, and participate in MDT meetings, collaborating with other health professionals. - Develop and implement care plans emphasizing person-centered care and evidence-based practices. - Partner with clinical teams to manage older adults with complex health needs effectively. - Contribute to the development of standards, policies, and protocols aimed at improving clinical outcomes. - Maintain strong communication with patients, families, and healthcare teams to ensure coordinated care. - Promote quality and safety initiatives and participate in evaluating interventions and patient outcomes. Requirements - Registered Nurse in New Zealand with a current Annual Practicing Certificate. - 4-5 years of nursing experience, specifically in gerontology or long-term conditions within New Zealand. - Completed a Postgraduate Diploma in Nursing (or equivalent) and currently working towards a Master's in Nursing. - Advanced clinical assessment skills in gerontology and care for older adults. - Experience in both hospital and community settings, particularly within aged residential care. - Demonstrated leadership abilities, including experience in coaching and mentoring nursing staff.
