About the Role We are excited to announce an opening for a Needs Assessor Service Coordinator at Lakes NASC, based in either Rotorua or Taupo. In this role, you will join our dedicated and multi-disciplinary team, working in a fast-paced and dynamic environment. Your primary responsibility will be to conduct holistic assessments of our clients' needs and coordinate support services for older adults with health and disability-related needs, as well as for younger individuals with chronic long-term health conditions. You will also engage with clients who have recently been discharged from the hospital and those in palliative care. Our goal is to ensure that our clients can continue to live safely at home or assist them in transitioning to residential care when necessary. This position will involve supporting clients throughout the Lakes region, including Rotorua, Mangakino, Taupo, and Turangi. About You We are looking for candidates who possess the following qualifications and skills: - A New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist, or Nurse - Current New Zealand Annual Practicing Certificate in the appropriate scope - Relevant professional experience in your field - A full New Zealand Driver’s License You should have proven assessment and clinical reasoning skills, alongside experience in collaborating with clients and their whānau to meet their support needs. Excellent communication skills and a track record of building positive relationships are essential.
