About the Role We are seeking an experienced social worker who is passionate about supporting Tangata Whai Ora (seekers of health) as they navigate their health journeys. This role offers a salary range of $77,087 to $107,600. The successful candidate will work within the inpatient service at Hutt Hospital, providing assessment, diagnosis, stabilization, treatment, and rehabilitation to patients with urgent medical needs and chronic conditions. You will collaborate across all wards, assisting patients who present with complex health and social needs and helping them manage these challenges within their communities. Your responsibilities will include conducting psychosocial assessments in partnership with patients and their whānau, working closely with allied health, nursing, and medical colleagues to facilitate safe and supportive hospital discharges. You will address a variety of referrals, including complex social issues and adjustments to new diagnoses, while also supporting discharge planning. About the Team/Service/Location Our team comprises 15 dedicated social workers based at Hutt Hospital, providing services to patients and their whānau across inpatient, outpatient, and community settings. We conduct psychosocial assessments for individuals of all ages and provide holistic social work interventions. This role is supported by a friendly and experienced adult social worker team, ensuring a nurturing work environment. About You We are looking for a highly motivated individual who is passionate about assisting patients and whānau as they navigate their health journeys. You should be adaptable and thrive in a fast-paced environment, with the ability to work collaboratively as part of a multi-disciplinary team and with external agencies. To be successful in this role, you will need excellent documentation, communication, and time management skills, as you will be working with a diverse range of patients who have significant health needs. This is an exciting opportunity to engage with various clinical areas. Requirements To be considered for this position, you should have the following qualifications and attributes: - NZ Registered Social Worker with a current annual practicing certificate - A minimum of 3 years of social work practice - A current full NZ driver's license - Experience in completing psychosocial needs assessments and developing robust plans - Familiarity with the legal requirements of healthcare and social work - A clear understanding of the health needs of Māori and Pacific Island communities - Therapeutic social work counseling experience (desirable) - Previous experience in a health setting and a good understanding of health social work (desirable) How to Apply If you are interested in applying for this role, please submit your application through our online careers portal by 3 August 2025.
