    Location: Kapiti Health Centre Contract: Fixed Term, Part Time (0.8 FTE / 32 hours per week) to cover parental leave period - approximately October 2025 to April 2026. About the Role We are pleased to announce an opportunity for a community social worker to join our Kapiti Community ORA Team. This position is community-based and part of a multidisciplinary team that collaborates to help patients achieve their independence at home and meet their functional goals. We are seeking a social worker who can leverage their skills throughout our service, employing a strengths-based approach to conduct comprehensive assessments and implement meaningful interventions. This role offers opportunities for professional development aligned with the service's objectives. About the Team/Service/Location The Kapiti Coast is a wonderful place to live and work. Our supportive team comprises diverse professionals, including Dietitians, Speech Language Therapists, Occupational and Physiotherapists, and Nurses, all working together to achieve the best outcomes for our patients. We are fortunate to have an onsite Geriatrician, allowing us to provide both compensatory and rehabilitative assessments and interventions for the people on the Kapiti Coast. About You We are looking for a competent and approachable professional who excels in teamwork and appreciates the unique contributions each discipline makes to health and wellbeing. As a social worker, you will support individuals and families through timely health and risk assessments. You should be skilled in risk assessment and therapeutic intervention, with an understanding of how changes in health status affect individual and family wellbeing. Familiarity with age-related illnesses, including dementia, and local and regional support networks is essential. You will need to manage a busy caseload efficiently and facilitate connections for patients with appropriate community support. As a confident social worker, you should be adept in conflict management and familiar with de-escalation strategies. Building and maintaining delicate relationships while working respectfully with individuals from various backgrounds, cultures, and value systems is crucial.
