About the Role
We are looking for a Health Care Assistant with community experience to join our Community Hub Rapid Response team. This role is essential in providing a multidisciplinary approach to patient care, with the primary focus on hospital avoidance, early supportive discharge, and non-acute rehabilitation within a homecare environment.
In this position, Health Care Assistants work under the guidance of registered health professionals to deliver care and support to patients in their own homes, facilitating Early Supported Discharge and Hospital Avoidance initiatives.
About the Team/Service/Location
Our Rapid Response team is dedicated to ensuring that patients receive timely and effective care in their homes. As part of this team, you will collaborate with various healthcare professionals while supporting patients and their families during their recovery journey.
About You
We are looking for someone who has the ability to work collaboratively with a diverse group of healthcare professionals. The ideal candidate will be capable of practicing independently and leveraging their previous experience to provide patient-centered care that promotes independence.
Key requirements for this role include:
- Previous community experience or a strong passion for providing care that supports individuals in their own homes
- The ability to work independently and utilize initiative and assessment skills to monitor and report changes in health status
- A commitment to delivering safe and effective patient-focused services
- Strong interpersonal skills to engage with patients and their whānau and family
- A current full New Zealand Driver's Licence

Salary Range
This position is governed by the terms and conditions of the Nursing Collective Agreement, and the salary will be determined according to the relevant pay scale and based on experience.
