About the Role
We are currently seeking a Health Coach to join our Access and Choice service on a part-time basis (0.5 FTE). This role follows a fortnightly schedule, with working days of Tuesday and Wednesday one week, and Tuesday, Wednesday, and Friday the following week, from 8:30 am to 5:00 pm. The position will be based at the Golden Bay Community Health Centre in Takaka.
In this role, you will provide free health and wellbeing support to individuals registered with their GP practice. You will work collaboratively with health professionals to enhance health and wellbeing outcomes, particularly in areas where lifestyle changes can have a significant impact.
Your responsibilities will include:
- Supporting adults aged 18 years and over
- Collaborating with the general practice team and acting as a valuable resource
- Demonstrating strong cultural awareness to effectively assist our diverse community
Requirements
To be considered for this role, you must meet the following criteria:
- Eligibility to work in New Zealand, which includes having New Zealand residency or a valid work permit
- A Level 4 New Zealand Certificate in Health and Wellbeing (NZCHW) or an equivalent qualification
- Experience as a health professional or Support Worker
- A satisfactory police check as part of the employment process
- A full New Zealand driver's licence
