About the Role Supportlinks is our Needs Assessment and Service Coordination (NASC) service, which facilitates access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district. We cater to individuals with acute or short-term needs, those with long-term health conditions, and older adults or kaumatua experiencing age-related health issues. In this position, you will be an integral part of the OPAL Community Services team, working throughout the MidCentral District. You will collaborate with a supportive transdisciplinary team that includes geriatricians, occupational therapists, physiotherapists, social workers, Māori connectors, and allied health assistants. We currently have a permanent full-time position (1 FTE) available in our Palmerston North office. We seek an enthusiastic, energetic, and resilient individual with a proactive "can do" attitude. We welcome applications from candidates who hold an annual practicing certificate in nursing, occupational therapy, physiotherapy, or social work.
