About the Role Supportlinks is our Needs Assessment and Service Coordination (NASC) service, which facilitates access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district. We cater to individuals with acute or short-term needs, those with long-term health conditions, and older adults or kaumatua experiencing age-related health issues. In this position, you will be an integral part of the OPAL Community Services team, working throughout the MidCentral District. You will collaborate with a supportive transdisciplinary team that includes geriatricians, occupational therapists, physiotherapists, social workers, Māori connectors, and allied health assistants. We currently have a permanent full-time position (1 FTE) available in our Palmerston North office. We seek an enthusiastic, energetic, and resilient individual with a proactive "can do" attitude. We welcome applications from candidates who hold an annual practicing certificate in nursing, occupational therapy, physiotherapy, or social work. About You We are looking for candidates who possess the following qualifications and skills: - Ability to adapt to a changing health environment - A mana-enhancing and restorative approach to practice - Experience working effectively within a transdisciplinary team, leveraging both individual and professional skills - Knowledge and established relationships with iwi/Māori providers in the district - Familiarity with community resources and networks - Proficiency in using a laptop and willingness to learn new assessment software - A strong desire to assist clients and their whanau in achieving their goals - Effective communication skills with individuals from diverse backgrounds and cultures - Strong written, oral, and interpersonal communication abilities - Excellent organizational and time management skills - A full, current driver's license Working at Health New Zealand At Health New Zealand | Te Whatu Ora, we are dedicated to providing excellent healthcare services to the people of New Zealand.
