Clinical Coordinator, Nelson Early Supported Discharge Team, Permanent Part-time

About the Role
In your role as Clinical Coordinator on the Early Support Discharge Team, you will take on several key responsibilities. You will lead the team while also engaging in your clinical duties, assisting clients in regaining their mobility and independence after their hospital discharge.
You will work collaboratively within a multidisciplinary team to provide person-centered care in both home and community settings. Your expertise will be vital in assessing, planning, and implementing individualized treatment programs to ensure the best possible outcomes for our clients. About the Team/Service/Location
The Nelson Tasman Early Support Discharge Service is dedicated to helping clients transition smoothly from hospital to home with the appropriate support. Our team works closely with clients, their whanau, and healthcare providers to enhance recovery outcomes and promote independence within the community. About You
To succeed in this role, you should be a registered health professional with a minimum of five years of clinical experience. Your background should include working alongside other professionals and participating in multidisciplinary team processes, as well as monitoring service delivery.
We are looking for someone with excellent interpersonal skills, a proactive attitude, and a strong focus on quality. As a capable leader, you will also be an enthusiastic team player who can bring innovative ideas to the table. Experience in projects or initiatives that incorporate the principles of Te Tiriti and apply Matauranga Maori and Kaupapa Maori approaches in healthcare settings will be advantageous.
