About the Role
We are seeking a permanent full-time Social Worker to join our Community Allied Health Services (CAHS) Team in Tauranga. This position requires a minimum of three years' experience in a health-related setting.
In this role, you will deliver social work services to clients in their homes and the community. Based at our CAHS office in central Tauranga, you will cover a broad geographical area in the Western Bay of Plenty. Our Allied Health team adopts a client-centered, enablement approach to work with individuals with physical health needs. Our primary goal is to maximize client safety and functional independence, enabling them to achieve their wellness goals and remain in their own homes.
A significant aspect of this position involves establishing and leveraging professional networks to address and meet clients' needs effectively. You will also be responsible for developing a comprehensive understanding of health systems and services, which will assist clients in navigating these resources.
About the Team/Service/Location
The CAHS team comprises allied health assistants, dietitians, occupational therapists, physiotherapists, social workers, and speech language therapists.

About You
The ideal candidate will demonstrate strong communication skills and have a proven ability to build connections within the wider community. Excellent time management skills are essential, as you will be independently managing a diverse caseload and conducting assessments with an interprofessional perspective. Collaboration with other professionals and the direction of an Allied Health Assistant will be integral to your work.
