    About the Company At Mahitahi Hauora, we are a not-for-profit charitable trust dedicated to improving health outcomes for our Tai Tokerau population. We work collaboratively with community organizations, primary healthcare providers, and key partners to support general practices, Māori health providers, and social sector entities. Our mission is to ensure that whānau and communities can achieve self-determined well-being, access the services they need, and lead long, healthy lives, all underpinned by the principles of Te Tiriti o Waitangi. About the Role We are currently seeking a Registered Nurse (RN) Practice Facilitator for a permanent position in Whangarei, with a work commitment of 0.1 FTE. In this role, you will use your clinical expertise to advance patient outcomes from a systems perspective rather than direct patient contact. As part of our team, you will support various practices and providers across the greater Whangarei region. Your clinical knowledge will play a crucial role in implementing a range of programs within general practice aimed at improving health outcomes, including immunization, management of long-term conditions, and cervical screening. The primary healthcare environment is continuously evolving to meet the needs of individuals and whānau, and your role will be essential in ensuring efficient and smooth practice function. You will collaborate with general practice teams to enhance models of care that increase access to services and improve patient outcomes. Your understanding of clinical models and pathways will be instrumental in facilitating change management related to quality and system improvement. You will also work with our wider team to support workforce capability and development, manage new program rollouts, and improve referral pathways and access to services for our Tai Tokerau enrolled population. This role offers a variety of challenges and opportunities for growth, allowing you to leverage collective strengths within the team. Requirements To be successful in this position, we are looking for someone who is relationship-driven, passionate about improving patient outcomes, and enthusiastic in their approach. Flexibility and innovation in your work will be key. Candidates should have a solid understanding of the primary healthcare and community environment, which will facilitate a smooth integration into the role. Comprehensive orientation and training will be provided to support your success. To be considered for this role, you must meet the following criteria: - Be a Registered Nurse with a minimum of 3-5 years of post-graduate experience - Demonstrate proficient-level experience or PDRP - Be comfortable with facilitation, change management, and ideally, education - Have experience in supporting and implementing services and/or programs - Possess advanced computer literacy in the MS Office 365 suite - Show evidence of the ability to develop and manage relationships with healthcare professionals - Be a confident communicator, both verbally and in writing - Hold New Zealand residency or citizenship Please note that all employees are required to be fully vaccinated for COVID-19.
