    About the Company At Hawkes Bay Fallen Soldiers Hospital and Central Hawke's Bay, we are dedicated to providing exceptional healthcare services to our community. Our Early Supported Discharge Service, known as Hoki ki te Kainga, embodies our commitment to the principles of Te Tiriti o Waitangi, as we strive to create a health system that serves all New Zealanders effectively. About the Role We are seeking an enthusiastic and motivated Occupational Therapist to join our Hoki ki te Kainga team. This is a new and exciting opportunity to work alongside our Hastings team while providing support to the Central Hawke's Bay community for three days a week. On the remaining two days, you will be part of an interdisciplinary early supported discharge service, assisting individuals as they transition from hospital to home. In both capacities, you will play a crucial role in helping clients maintain their independence in their own homes. Each day will present new challenges and opportunities for learning. You will engage in collaborative goal setting with clients and their families, design individualized care plans, provide equipment solutions, and conduct cognitive assessments. Your focus will be on maximizing independence, transforming everyday moments into valuable rehabilitation opportunities. About the Team/Service/Location Hoki ki te Kainga is a community-based service dedicated to supporting older adults as they transition from hospital care to their homes. Our mission is to provide timely and appropriate support, enabling individuals to recover safely and regain their independence. The Central Hawke's Bay component of our service offers a diverse range of experiences and the chance to enhance your occupational therapy skills while embracing an interprofessional approach to healthcare delivery. You will have the opportunity to work collaboratively with patients in the CHB inpatient ward and clients in the community. About You We are looking for an Occupational Therapist who meets the following criteria: - New Zealand Registered Occupational Therapist with a current annual practicing certificate - Member of Occupational Therapy New Zealand - Professional Association (desirable) - Holds a full and current driver's license - Enable NZ Accredited Assessor status (or willing and able to attain this) In addition, we seek candidates with the following key attributes and skills: - Experience in rehabilitation, orthopaedic, geriatric, or community settings - Strong clinical reasoning and assessment skills - A passion for improving health outcomes and supporting independence - An adaptable approach with a strong desire to help clients and their families achieve their goals Working at Health New Zealand At Health New Zealand | Te Whatu Ora, we are committed to ensuring excellent healthcare for the people of New Zealand.
