About the Role
We are currently seeking an Occupational Therapist/Case Manager with experience in Mental Health to join our team within the Tauranga Mental Health Services for Older People Community team. This is a permanent position, offering 0.75 Full-Time Equivalent (FTE), which equates to 60 hours per fortnight.
In this role, you will play a vital part in delivering high-quality care to the growing population in the Bay of Plenty. Your responsibilities will include collaborating with the Tauranga community MHSOP services, the inpatient MHSOP ward, health partners, referred clients, their families and whānau, as well as the wider community. Key tasks will involve conducting effective triage and assessments, facilitating and implementing planned treatments, and maximizing health and independence within the community. Specific duties include:
- Providing advice and assistance
- Resolving mental health crises
- Facilitating transitions along the care pathway for service users and their families
- Linking consumers to primary care providers and appropriate community agencies

About You
The ideal candidate will possess a thorough understanding of mental health conditions, the Mental Health Act, and the PPPR Act, as well as a solid foundation in capacity assessment and a variety of therapeutic modalities. A minimum of three years of postgraduate experience in mental health is required, with a preference for candidates who have experience in older persons' mental health.
